Please send the prescribed application form (downloadable from here) along with one document confirming your identity (e.g., copy of driver's license or passport) enclosed in an envelope marked with "Request for Disclosure" on the front to the address below (limited to postal mail due to personal information protection considerations).
For matters related to specific personal information, please clearly indicate "My Number Administration" after the association's name in the address.

(2) Disclosure Fee

Please enclose stamp(s) for 84 yen as postage for the reply envelope.

(3) Response Method

We will respond in writing to the address specified in the application form.

3. Corrections and Amendments to Personal Information

In case you wish to request corrections, additions, deletions, or other changes to personal information (held personal data) by our association, please follow the procedure below:

(1) How to Make a Request

Please enclose the designated application form (downloadable from here) along with one document confirming your identity (e.g., copy of driver's license, passport) in an envelope marked "Request for Changes" on the front. Please send it via postal mail to the address below (limited to postal mail due to personal information protection considerations).
For matters related to specific personal information, please clearly indicate "My Number Administration" after the association's name in the address.

(2) Response Method

We will respond in writing to the address specified in the application form.
